Barnett's research focuses on the cost-effectiveness of healthcare inventions, determination of the cost of healthcare in the absence of billing data, and economic modeling in support of healthcare policy decisions. He is a health economist at the VA Center for healthcare Evaluation, the VA Cooperative Studies Program Coordinating Center, and the Treatment Research Center at the University of California, San Francisco.
He is also Consulting Assistant Professor in the Department of Health Research and Policy at Stanford University School of Medicine. His health services research focuses on the cost-effectiveness of treatments for substance abuse disorders. He is applying methods used to evaluate medical care interventions to the substance abuse field. A model of the effect of methadone maintenance on healthcare costs and survival determined the treatment is at least a cost-effective as other healthcare interventions provided to injection drug users. This work is being extended via a dynamic model of the HIV epidemic being developed in collaboration with Stanford Professor Margaret Brandeau. They are also combining the results of a meta-analysis of trials comparing buprenorphine to methadone with the HIV model to determine the potential cost-effectiveness of buprenorphine maintenance.
Barnett is assisting investigators in the University of California, San Francisco Treatment Research Center evaluate a medication management intervention for HIV-positive drug abusers, among other interventions. He is assisting a VA trial testing if a matching strategy improves hospital and community- based residential treatment for substance abuse patients with psychiatric disorders.
He has lead a team that completed an economic analysis of data from the VA Non-Q-wave Infarction Strategies in Hospital (VANQWISH) trial, which demonstrated that a conservative, ischemia-guided strategy was safe and effective for management of non-Q-wave myocardial infarction. The conservative strategy was shown to be more cost-effective (click here for article), a result that is helping to shape clinical opinion on the best treatment for heart attack (click here for editorial). He also serves as the VA economist on a trial evaluating PTCA in the context of intensive medical therapy in the treatment of all but the very highest risk patients with coronary heart disease. He is leading a team that is studying whether Positron Emission Tomography is cost-effective enough to justify its adoption for the diagnosis lung cancer.

Shuo Chen is a Research Associate in the VA Health Economics Resource Center and the Palo Alto Cooperatives Studies Program Coordinating Center. His research has included studies of the cost-effectiveness of management of cardiac disease, and the evaluation of the cost of VA substance abuse treatment programs. He is the principal staff on the HERC average cost product line. He has his Ph.D. in Social Demography from Kansas State University where he studied cancer morbidity among farmers.

Dr. Phibbs is a health economist at the Center for Health Care Evaluation and the Cooperative Studies Program Coordinating Center, a Consulting Associate Professor in the Department of Health Research and Policy, Stanford University School of Medicine, and a Research Associate, Center for Health Policy and Center for Primary Care and Outcomes Research, Stanford University. His own VA research projects have focused on the demand for VA healthcare and how travel distance and the availability of non-VA alternatives influence veterans use of VA care. In more general terms, his research focuses on the hospital costs and competition. Much of this work has looked at how travel distance effects hospitals, and methods for defining hospital markets. Dr. Phibbs also has a long standing interest in neonatal care. Since coming to VA he has continued to conduct some neonatal related research with non-VA funding. This work is currently focusing on how the competition-driven expansion of neonatal intensive care units has effected neonatal mortality.

Mark Smith performs research in three areas: cost-effectiveness of health interventions, micro-costing methods, and the economic impact of trauma and violence. He is Associate Director of the VA Health Economics Resource Center (HERC), where he leads the development of guidelines for research using micro-costing methods. He is also a member of the VA Cooperative Studies Program (CSP), serving as health economist for clinical studies. Beyond VA, Smith is an associate of the Center for Primary Care Outcomes and Research (PCOR) at Stanford University.
A major area of Smith’s research is the economic impacts of trauma and violence. Using data from Washington State, Smith quantified the impact of violent abuse on the earnings of poor women. A later study investigated the impact of abuse on poor women's use of public assistance programs. Smith collaborates with the VA National Center for PTSD in research on military sexual trauma (MST). He also serves as health economist for VA CSP trial #519, which investigates integration of smoking-cessation therapy with PTSD therapy.
A second research interest is the economics of telehealth interventions. Smith is currently estimating the potential cost savings from using telehealth in the treatment of veterans with spinal cord injuries who develop pressure ulcers. He also directs the economic analysis for a clinical trial of telephone follow-up for veterans recently discharged from inpatient treatment for PTSD.
Prior to joining VA, Smith studied pharmacoeconomics and health services at The Medstat Group (now Thomson Reuters). His research there investigated the cost implications of using alternative antipsychotics, health promotion programs to reduce health risks or hospitalization, and the role of price increases in explaining rising national drug spending.

I'm a health economist with the Palo Alto VA. At the VA, my primary position is with the Health Economics Resource Center, but I am also affiliated with the Center for healthcare Evaluation and the Cooperative Studies Coordinating Center. The Palo Alto VA has a close relationship with Stanford University. In particular, I am affiliated with the Center for Health Policy (CHP) and the Center for Primary Care and Outcomes Research (PCOR).
In addition to my VA work, I am interested in the evaluation of consumer health information and the cost of research administration (including IRBs). Other areas of interest include cost-effectiveness analysis, quality of life measurement, and access to care for multicultural populations.
I received my Ph.D. in Health Economics from the Health Services and Policy Analysis Program at the University of California, Berkeley in 1999.

Dr. Yu's research interests include determination of healthcare costs, cost-effective analysis, risk adjustment method, healthcare financing policy and healthcare delivery structure under different institutional structures, cultural backgrounds, and economic capacities.
Dr. Yu is a health economist at the VA Health Economics Resource Center, VA Center for healthcare Evaluation, and the VA Cooperative Studies Program Coordinating Center. He is also a fellow at the Center for Health Policy and Center for Primary Care Outcomes and Research at Stanford University.
In determination of healthcare costs, he investigated costs of nursing homes in South Carolina as well as use of hospital service and intensive care for the elderly. As Research Faculty of the healthcare Financing Administration (HCFA) Research Data Assistance Center, he taught Medicare reimbursement methods, and prepared training databases for health services researchers on how to identify Medicare expenses and hospital costs in HCFA files.
He is currently leading the product line at the VA Health Economics Resource Center to evaluate the Decision Support System, a cost accounting system developed to identify costs of healthcare services provided by the VA. He recently analyzed effects of age and chronic diseases on healthcare costs. He is currently evaluating cost-effectiveness of treatment strategies for HIV/AIDs patients who have failed second runs of anti-retrovirals.
Dr. Yu has analyzed healthcare costs and utilization during the last year of life, including differences in hospice use, place of death, and cause of death between decedents enrolled in a managed care plan and a fee-for-service plan. He also compared Medicare expenses between decedents who used hospice services during the last year of life with those who did not. Dr. Yu is expanding this research to the VA Health Care System. He has studied long-term care use near the end of life among VA patients.
In international studies, Dr. Yu has provided consultation on healthcare financing policy, structure of the community healthcare system, and reimbursement methods for hospital services in China in separate projects funded by the United Nations Development Programme and the Ford Foundation. He is currently coordinating a China-US post- doctoral training program funded by the National Institute of Health at Stanford. His recent research is comparing risk diversification of the employer-based health insurance systems between US and China.

Dr. Hendricks is Chief of the Economics Research Section at the Center for Health Quality, Outcomes and Economic Research (CHQOER) and Associate Professor of Health Services at Boston University School of Public Health. She is currently the principal investigator for a VA service directed project to evaluate variations in patient severity as they relate to VA budgets and co-PI (with Gary Nugent of the Cincinnati VAMC) on an evaluation of VA costs. She is also co-investigator on an NCI-funded project at Massachusetts General Hospital to study the cost-effectiveness of home care for cancer patients with febrile neutropenia. She is currently the PI on a Congressionally mandated evaluation of VA's long term care programs. She is also a member of an advisory committee examining budget issues related to VA's research support budget. She consults to other CHQOER researchers on economic analyses related to telemedicine, ambulatory care, and costs of care in VHA. She serves on dissertation committees for Ph.D candidates at Brandeis University's Heller Graduate School of Social Welfare. She has taught Cost Effectiveness and Decision Analysis at the Boston University School of Public Health.

Dr. Bennett's work addresses costs of care for cancer therapies, with a focus on new therapeutics, devices, and technologies. He currently is leading an effort, in collaboration with the International Blood and Marrow Transplant Registry, to evaluate costs of stem cell transplantation at 18 centers in the United States. He also has led early efforts in this field, with extensive publications from the University of Nebraska experience as well as in conjunction with newer approaches including allogeneic peripheral blood stem cell transplantation and adjunctive use of hematopoietic colony stimulating factors. Other cost studies involve maximal androgen blockade therapy for prostate cancer, and amifostine for ovarian cancer, and liposomal doxorubicin for HIV-related Kaposi's sarcoma.

My research focuses on the use of financial incentive for affecting patient/healthcare consumers behavior. For example, the use of financial incentives for increasing cadveric organ donation rates has just been instituted in Pennsylvania. My research begins with an economic model to determine the most effective and efficient method for financial compensation. In addition, ethical analyses will explore the use of these financial incentives in terms of whether they may be coercive, exploitative or commodify the human body.
In another line of research, I am exploring the relationship between primary care utilization and hospitalization rates for ambulatory care sensitive conditions.

Kathleen Carey, Ph.D. is an economist with the VA Management Science Group. Her primary research interest is the study of hospital and other healthcare costs with particular emphasis on comparing the performance of VA with the private sector. She has expertise in applying state-of-the-art econometric models to VA and nonfederal hospital cost analyses. Currently, she is leading the MSG effort to examine the effects of managed care on VA healthcare costs and comparing that to managed care effects outside the VA.

Previous studies have attempted to compare the costs of VA healthcare services to the private sector. Most found that VA costs were somewhat lower but differences in accounting systems focus i.e. inpatient, and populations between the two systems limited their usefulness. The comparison of costs is an important question with increased interest in contracting out some or all of veteran=s healthcare. The primary objective of this proposal is to compare the cost of VA services to Medicare costs for those same services under: a) fee for service, and b) capitation. To circumvent the impact of volume on unit cost and cost shifting, we intend to capture and compare all work performed over the course of an entire fiscal year at six VA medical centers (Albuquerque, Birmingham, Cincinnati, Kansas City, Milwaukee, and Providence). This includes overhead distribution i.e. malpractice and depreciation, and Medicare non-covered services such as pharmacy, dental, and domicilliary. In order to facilitate comparison, audits of coding accuracy, utilization review, and severity have been incorporated into the study design. The VistA database will be used to generate HCFA 1500 and UB 92 pseudo bills. Inpatient reimbursement will be determined by processing discharges, in accordance with ICD-9 CM criteria, though 3M Encoders that have been installed at each site and programmed with area specific Medicare reimbursement data. Ingenix (Salt Lake City, UT) will act as the fiscal intermediary and process outpatient facility charges and professional fees using CPT-4 and HCPCS codes extracted from the VistA database. At the completion of the study it will be possible to compare cumulative costs to cumulative reimbursement by medical center and major sub-category i.e. medicine, nursing home, pharmacy or prosthetics.

the UCSF Department of Epidemiology & Biostatistics and Medicine, the UCLA School
of Public Health, Department of Health Services, and is a senior scientist at
RAND. His investigations are in the areas of healthcare delivery and the impact
of provider incentives on the quality of services. Prior to joining the faculty
at UCSF and UCLA, he worked in Asia for six years including three years as a
staff epidemiologist / medical officer in WHO. Since coming to the VA, Dr. Peabody
has continued to work overseas in Latin America, Eastern Europe and Asia and
has developed an active research program within the VA. This work has focused
on measuring the quality of care across the VA and comparing the cost and quality
of care with other nearby healthcare systems. Dr. Peabody also has an active
clinical practice.

Dr. Robert Rosenheck is the Director of VA's Northeast Program Evaluation Center, Director of the Evaluation Division of the National Center for Posttraumatic Stress Disorder (PTSD); and Professor of Psychiatry and Public Health at Yale where he is also Director of the Division of Mental Health Services and Outcomes Research. He is a nationally known mental health service researcher who is a leader in cost-effectiveness studies of behavioral health interventions. He has been Co-Chairman of three multi-site VA Cooperative Studies on the cost-effectiveness of new medications and is responsible for the monitoring and evaluation of several hundred specialized VA programs devoted to the care of veterans suffering from PTSD, homeless veterans, and veterans who suffer from severe and persistent mental illnesses. His cost-effectiveness studies of VA's specialized PTSD programs have played a major role in transforming the way PTSD is treated in VA, and he has published widely on the subject outcomes monitoring and service system evaluation. He has been a prime architect of national VA collaborative programs with both the Department of Housing and Urban Development and the Social Security Administration. He also directs the client-level evaluation of the 5-year, 18-site ACCESS program for homeless mentally ill Americans, for the Substance Abuse and Mental Health Services Administration of the Department of Health and Human Services. He has published more than 300 scientific papers.

I am currently involved in 2 active projects, and the Ischemic Heart Disease Quality
Enhancement Research Initiative (IHD QUERI) Research Coordinating Center as the
Translation Coordinator.
The first major project I am Principal Investigator on is a translation project
to improve the quality of care offered to veterans with ischemic heart disease
(IHD). In this study, we are launching and monitoring the effects of interventions
designed to speed up the rate of quality improvement. We will also be looking
at costs of the interventions, and costs of secondary prevention for IHD in VA.
We are actively working with 3 VISNs and approximately 20 VAMCs.

Jean Yoon is a Health Economist in the VA Health Economics Resource Center and the Cooperative Studies Program Coordinating Center. Her main research interests involve care for chronic diseases, cost-effectiveness, and socioeconomic and racial disparities in health. In her most recent research, she examined adherence to prescription drugs and its relationship to cost-sharing for drugs and health outcomes. She previously worked at UCLA, UCSF, and the Health Resources and Services Administration studying access to primary care, the safety net, and disparities in quality of cancer care. She received her Ph.D. in health services from UCLA.
